9. Research Forms and Instructions

9.1 Preliminary Project Approval Form

Must be filled out and submitted to OSP if the Principal Investigator wishes to have an Project Information Sheet established prior to the receipt of an a complete award document. 

REQUIRED SIGNATURES.
    • Principal Investigator and any co-Principal Investigators
    • the designated official for the department, division, institute or center concerned
    • the Dean or, if none, the cognizant Vice-President for the organizational unit with which the PI is affiliated


 

9.2 Conflict of Interest Disclosure Form

The Conflict of Interest Disclosure form is maintained by the Office of the Vice President of Research Integrity and Compliance, and is linked from this site as a courtesy to researchers by the Office of Sponsored Projects.  If there any doubts as to whether you have the latest version of the form, please telephone the University Conflict of Interest Office at (801) 581-6351.

This form should be filled out and submitted to OSP together with the Document Summary Sheet according to the procedures specified at the following location www.research.utah.edu/integrity/conflict/forms.html and in accordance with the University of Utah Conflict of Interest Policy.

9.3 Deposit Definition Checklist Form

The Deposit Definition Checklist Form is needed to ensure compliance with tightened Federal cost accounting standards, and is meant to ensure that no restricted grant/contract funds are deposited into non-restricted Funds.

Any deposits over $5,000 made with Income Accounting (the Cashier) to funds 2000, 6000, 6100, 6300, 6400 and 8700 must be submitted with the Deposit Definition Checklist Form in addition to the Departmental Deposit Form.  (To obtain a Departmental Deposit Form, please call Income Accounting at (801) 581-7344.)  Funds will be credited to a holding account until both forms are received and reviewed, to ensure that only non-restricted funds (gift funds with no restrictions attached by a sponsor) are deposited into the funds listed above.

Gift funds should always, and with no exceptions, be routed through the University's Development Office on Main Campus for review.   The Development Office works in tandem with the Office of Sponsored Projects and Income Accounting to ensure that gift funds are properly identified and reviewed.

The Deposit Definition Checklist Form is packaged with a memorandum from the Vice-Presidents of Development and Research and also with a short page of instructions.

9.4 Document Summary Sheet

Submission of the Document Summary Sheet to Sponsored Projects is required with competing new or competing continuation project proposals or contracts, and is intended to gather summary and specific project information, institutional approvals (signatures), and information pertaining to regulatory requirements.  The form is used by the Office of Sponsored Projects during pre-submission proposal and new contract reviews, and lays the groundwork for processing and setting up an award.

    • Principal Investigator and any co-Principal Investigators
    • the designated official for the department, division, institute or center concerned
    • the Dean or, if none, the cognizant Vice-President for the organizational unit with which the PI is affiliated


 

9.5 Green Card Application (for Clinical Investigations)

The Green Card Application form is the responsibility of the University Hospital Patient Accounting office, and is hosted as a courtesy to researchers by the Office of Sponsored Projects on the OSP website.  Researchers and staff should ensure that the form is the latest version available.  If there any doubts on this point, please email the Study Account Representative in Patient Accounting.

To be filled out and submitted at the beginning of each clinical investigation.  The "Green Card" Application helps to ensure that the clinical investigation or department pays for protocol-related procedures instead of charging a patient out-of-pocket or billing a patient's health insurance company.  The "Green Card" looks like a green credit card and contains vital information (i.e., the green card account number, ARUP account number, the name of the clinical investigation, investigating department, doctor's or Principal Investigator's name, General Ledger or PeopleSoft activity number, and the clinical investigation coordinator's name, address and phone number).  Once the Principal Investigator receives a project number from Research Accounting, a project Green Card should be applied for and obtained before the PI starts seeing patients related to a clinical investigation project.

9.6 Guest Lecturer/Performer Agreement

The Guest Lecturer/Performer Agreement is maintained by the University's Accounts Payable office answering to the University's Controller's Office, and is linked from the OSP website as a courtesy to University faculty.

The Guest Lecturer/Performer Agreement should not be used to arrange services from independent providers that will be paid from a sponsored project.  In such cases, a Professional Services/Consulting Agreement and an accompanying  Employee/Independent Contractor Classification Checklist should be completed.

The Guest Lecturer/Performer Agreement may be used under infrequent circumstances to arrange for guest lecturers and entertainer performances.  Parties paid under the Guest Lecturer/Performer Agreement may not be paid from project chartfields, which fall within the 50000000 range, unless the project sponsor has explicitly agreed to pay applicable fees.  Please refer to University Policy and Procedures Manual 3-24 and 4-14 (and contact an OSP administrator if there are further questions) to gain a clearer understanding of how services paid under the Agreement differ from those contracted through the Professional Services/Consulting Agreement.

INSTRUCTIONS AND RELATED FORMS.  Instructions on completing the Guest Lecturer/Performer Agreement are included with the Agreement (please refer in particular to "Standard Processes", item I.D.1.).  Note that the terms of the Agreement specify payment of the honorarium or performer fee immediately following the lecture or performance.

Other forms involved in the lecturer/performer agreement and payment process include:

  • IRS Form W-9; may also need to be completed if the consultant or service provider provides their Taxpayer ID on the Agreement.
  • A Check Request; must be completed (see the Accounts Payable Check Request page), signed by the appropriate officials, and submitted to Accounts Payable in order to pay the service provider or consultant.  (An illustrative Check Request form is available with instructions on completing the Request.)

9.7 Interdepartmental/Intercollegial F&A Distribution Agreement

Fill out and submit to OSP with a Document Summary Sheet form if investigators on the project, including the Principal Investigator, come from more than one college or department.  The F&A (Facilities & Administration costs) Distribution Agreement provides a vehicle for all centers, institutes, departments and colleges represented by project staff to agree upon a method of sharing F&A payments made to the University.  If preferable, a similar agreement addressing the same concerns may be written and completed.

REQUIRED SIGNATURES.
    • Principal Investigator and any co-Principal Investigators
    • cognizant academic officials such as the Department Chair and Dean for each organizational unit through which the PI and each co-investigator plans to perform the work


 

9.8 Budget Worksheet

Filled out by the PI, and submitted for review to Sponsored Projects, when an R01, R03, R21, SBIR/STTR subgrant, or RFA-specified "modular grant" application is selected for funding by the National Institutes of Health.

The budget form is needed to summarize planned project budgets to ensure compliance with tightened Federal cost accounting standards, and must be reviewed by a contract/grant administrator in the Office of Sponsored Projects.

9.9 Notice of Intent to Fabricate

Filled out by the Principal Investigator when supplies, equipment and/or other materials purchased from funds provided under a contract or grant will be integrated into a single, stand-alone piece of equipment with a cumulative cost of $1,000 or more, and the equipment will not be attached to a building.

9.10 Professional Services/Consulting Agreement

The Professional Services/Consulting Agreement, Employee/Independent Contractor Checklist, and the University of Utah Check Request are maintained by the University's Accounts Payable office answering to the University's Controller's Office, and are linked from the OSP website as a courtesy to researchers.

Used to describe consulting or services provided to a University project, and to enter into a contractual arrangement between the University and each consultant or service provider.  In some cases, review and approval by the Office of Sponsored Projects is necessary.

INSTRUCTIONS AND RELATED FORMS.  Instructions on completing the Professional Services/Consulting Agreement are included with the Agreement (please refer in particular to "Standard Processes", item I.D.1.).

Other forms involved in the independent consultant or service provider agreement and payment process include:

  • An Employee/ Independent Contractor Classification Checklist.  Submission with the Agreement is required.  (The Checklist is packaged for distribution in the same file as the Agreement, and contains instructions.)
  • IRS Form W-9; may also need to be completed if the consultant or service provider provides their Taxpayer ID on the Agreement.
  • A Check Request; must be completed (using either the online version or a printable template version), signed by the appropriate officials, and submitted to Accounts Payable in order to pay the service provider or consultant.  (An illustrative Check Request form is available with instructions on completing the Request.)

REQUIRED SIGNATURES--Professional Services/Consulting Agreement.  If an external contract/grant will be used to pay the consultant, the following parties should sign the Agreement:
  • consultant
  • Principal Investigator
  • Director, Sponsored Projects

If no external contract/grant will be used to pay the consultant, the following parties should sign the Agreement:
  • consultant
  • a project's responsible person
  • responsible person's Dean or Vice-President
